Before Your First Visit
You can make your first appointment by email by emailing the therapist of your choice from the "Contact Us" tab on the home page. If you prefer to make your appointment by phone, please call (972) 231-2555.
Your therapist will return your call within four hours.
Then, download and print the intake forms. These forms are found below. Please choose the forms specific to your needs. Adult clients will choose the Adult Intake Forms. If you are coming as a couple, married or not, choos Couples Intake Forms. If the client will be a child under 18 years of age, click on Child and Adolescent Intake Forms.
Complete the forms and bring them to your first visit.
There will be one form related to your insurance. Complete the top half and fax/email it to us prior to that visit. We will attempt to verify your coverage prior to your arrival.
By following these instructions you will have a smoothe and seamless intake.
