Before Your First Visit
You can make your first appointment by emailing me from the Contact Me tab.
If you prefer to make your appointment by phone, please call (972) 768-1124. I will return your
call within four hours.
Then, download and print the intake forms. These forms are found below. Please choose the
forms specific to your needs. Adult clients will choose the Adult Intake Forms. If you are coming
as a couple, married or not, choose Couples Intake Forms. If the client will be a child under 18
years of age, click on Child and Adolescent Intake Forms.
Complete the forms and bring them to your first visit.
There will be one form related to your insurance. Complete the top half and fax/email it to me prior to that visit. I will
attempt to verify your coverage prior to your arrival.
By following these instructions you will have a smoothe and seamless intake.
